How do I submit an attachment for a claim?

Health Plans: 

  • Horizon BCBSNJ

Workflow: 

  • Claim Status Inquiry

Click on the Submit Attachments link to add attachments to the claim.

The following fields are required on the Submit Claim Attachments screen:

  • Attachment Type
  • File Name

You must also read and check the box agreeing with the disclaimer before submitting an attachment.

Pre-Existing Attachment

You can only submit pre-existing condition attachments for local claims pended for more than 7 days that display a claim message or claim-line message relating to pre-existing conditions.

The following fields are required on the Submit Claim Attachments screen:

  • Attachment Type
  • File Name 

Notes:

  • The Submit Attachments link appears on the Claim Status Details screen only for local claims that are pended or finalized with a claim amount paid of $0.00.
  • Click on the Attachment Guidelines link to learn more about submitting attachments and view attachment types codes and definitions.
  • Click on More Attachments... to submit additional attachments.
  • You can submit up to 5 attachments per transaction.
  • Recently submitted attachments are not immediately available in Attachment History on the Claim Status Details screen.
